Masonic Villages at Elizabethtown is seeking a Home Health RN Case Manager. This is a Pool Opportunity.

What You'll Do:

As a Home Health RN Case Manager, you will carry out our Mission of Love by working independently with groups of clients to provide initial and on-going assessment of the client's skilled needs, implementing individual plans of care and make referrals to others disciplines and/or revise the plan as necessary. They also will provide ongoing consultation and education for patient/family member regarding all aspects of care. Our Home Health RN Case Manager are responsible for the following:

  • Complete initial and ongoing comprehensive assessments of the client's needs, including (b) Initiating the plan of care and revising as necessary
  • Provide skilled nursing care services and/or treatments requiring substantial and specialized nursing skill, including intravenous fluids and medications, urinary catheterization, and complex wound care.
  • Counsel the client and family in meeting nursing and related needs.
  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Prepare clinical and progress notes.

What You Definitely Have:

  • One year of nursing experience, preferably in Hospice, Home Health, or a Skilled Nursing Facility
  • Unencumbered State License as a Registered Nurse
  • Current CPR certification (Basic Life Support, In-classroom training)
