Masonic Village Home Health, Hospice and Home Care is hiring a Home Care LPN/Care Coordinator. This is a Pool opportunity.

What You'll Do:

As a Home Care LPN/Care Coordinator, you will carry out our Mission of Love by assisting the Home Care client(s) with navigating their healthcare to ensure wellness and optimum health of the client ie; making appointments, escorting them to their appointments, arranging Home Health/Hospice of their choice if appropriate, coordinating their medications with their pharmacy, providing general oversight and advocacy. Our Home Care LPN/Care Coordinator is responsible for the following:

  • Provide services in accordance with organizational policies and procedures and regulations that define scope of practice
  • Complete scheduled client visits as arranged. Document care that was provided.
  • Assist the client of client's family with providing resources with coordinating their care.
  • Carry out the support plan, including coordination of medication with pharmacy of client's choice, and pain assessments. Notify Home Care Manager with any changes or concerns.
  • LPN to work within their scope of practice.
  • Provide teaching to assist the client/client's family/primary caregiver in learning appropriate care techniques
  • Ensure good communication of information to client/client's family/POA.

What You Definitely Have:

  • Unencumbered State license as a Licensed Practical Nurse (preferred). 
  • One year of experience in Hospice, Home Health or a Skilled Nursing Facility (preferred). 
  • Current CPR certification (Basic Life Support, In-classroom training).
  • Good verbal and written communication skills. 
  • Able to travel throughout Lancaster County and work under limited supervision.
  • Able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least 2 flights of stairs and be able to stand and/or walk for a period of at least 2 hours. 
  • Valid PA driver's license and proof of motor vehicle insurance.
